Role Purpose:
Adferiad provides a floating support service across the County Borough of Bridgend; focusing on individuals referred to Adferiad who have history of repeated homelessness, very complex needs, experience of multiple disadvantage and for whom other services have not been successful in ending their homelessness. The emphasis of the project is to develop or sustain an individual’s capacity to live in their own accommodation and achieve increasing levels of independence. The project also includes supporting to live independently in their own homes.
This service is available between 9.00 am to 8.00 pm - Mon to Sun with specific hours of support determined by each client needs. Staff will be expected to be flexible and work across the hours and days of service delivery and will be part of a rota of on-call service for out of hours provision where necessary.
Under the direction of the Senior Service Manager & Senior Support Mentor, you will work as part of the team and lone work to support and improve the lives of those individuals by providing high quality and effective housing related support service. You will utilise person centred approaches to set goals, review and develop support plans that best meets the individual’s needs, which focusses on people’s strengths and sets goals.
