Crynodeb
The teams aim is to empower and promote individuals with a learning disability and complex needs to live meaningful lives. To adopt Positive Behaviour support approaches to reduce length of time in hospital and to support the prevention of inappropriate admissions or readmissions. This will be working within the home environment or appropriate care settings, alongside the wider Multi-disciplinary team. To support people with a learning disability and/or autism who display behaviour that challenges, including those with a mental health condition. Assess individuals with significant complex needs and / or challenging behaviours requiring individualised packages of care. The post holder will be involved in mentoring staff and students.
