Crynodeb
The attached Job Description provides the formal outline to the role we require from the Lancs &South Cumbria GPN Resource Pack v4 2018. It will need to be finalised in consultation with the successful candidate in line with their experience and skill level. Our primary focus for this role is the support of long-term condition patients through diagnosis, management and routine monitoring of their condition. We are looking for an enthusiastic nurse willing to use motivational interview techniques in their patient consultations. The successful candidate will demonstrate a self-motivated desire to keep their skill current and responsive to emerging developments in the care of diabetes patients. The new nurse will be able to empower the patient with a clear understanding of the challenges facing someone with this condition.
