We have an exciting opportunity to recruit a Housing First Practitioner for a minimum of 3 months in Liverpool, working 35 hours per week.
If successful, you will be responsible for providing and coordinating personalised, trauma-informed support and practical help to your own caseload of vulnerable individuals to reduce the risk of homelessness and ensure tenancy sustainment.
Your new role
- Managing your own caseload and establishing trusting relationships with individuals who are or have faced homelessness within temporary accommodation
- Carrying out risk assessments and safety planning, identifying any potential risk/safeguarding concerns
- Co-creating trauma informed and person centred support plans with positive, motivating and achievable outcomes to be reviewed regularly
- Enabling access to various voluntary and statutory community resources, services and opportunities
- Maintaining good relationships with landlords, stakeholders and other agencies to ensure high-quality wrap around support services are consistently provided
- Providing practical help and advice in relation to benefits and housing options
What you'll need to succeed
- Experience of working with vulnerable individuals facing homelessness, mental health, substance/alcohol misuse
- Good understanding of housing and homelessness legislation, safeguarding policies, the complexities of substance/alcohol use and how mental health needs can contribute to homelessness
- Experience of carrying out risk assessments and creating person-centred support plans using trauma-informed care & strengths based practice
- Good understanding of welfare benefits and ability to provide advice and practical help
- Excellent communication and interpersonal skills, confident lone working with good time management skills
