Main Tasks To plan and provide specialist nursing care and Nurse led clinics, support the care provision for patients with long term Neurological conditions (Epilepsy) Assess patients with complex specialist conditions and make judgements on appropriate action for specialised care plans/programmes of care. To assess, plan, implement and evaluate specialist nursing care, establishing mechanisms by which each patient has a coherent and comprehensive management plan. These plans will be developed with the patient and carer according to their needs and include referral to other disciplines and services observing the principles of holistic care. To recognize the effects of polypharmacy. Maintain highly developed skills for the delivery of specialised patient care requiring dexterity and accuracy. To provide expert and professional and clinical care advise to patients, carers and the MDT. Coordinates multi-disciplinary groups in specialist care. To attend and contribute to local, regional and national forums and networks. Promote and provide effective communication; with other professionals, patients, carers and with other care settings, where information maybe highly complex, sensitive and may be contentious. To maintain accurate, appropriate and concise patient and other appropriate records Aim to empower the patient with Epilepsy by supporting and promoting their involvement in decision making. To provide specialist advice and information to professional colleagues, patients, carers and the public.
