Base Location: Wilshaw House, Wilshaw Lane, Ashton-under-Lyne, OL7 9QG

The Role
An exciting opportunity has arisen to join the Home and Community Support Team as a Patch Co-ordinator providing support, supervision and line management to front-line staff who provide high quality care in the community, promoting and maximising a person’s level of independent living within their own home and the local community.
Shift times for this post are 07.00 13.50 and 15.10 22.00 over seven days a week, which includes some weekend and bank holiday working
About You
You will be able to support and offer practical advice to staff whilst they are on shift
You will be able to introduce newly referred people to the service and complete environmental and personal risk assessments
You will be able to liaise with internal and external agencies
You can use databases, Microsoft packages and virtual platforms, you will also be required to use CM 2000 scheduling system and update service user records
You can work well under pressure and are able to motivate and guide staff
You have excellent communication and interpersonal skills
Ideally you should have a level 3 Care Certificate or be willing to work towards one
You must be a car user for which a casual car allowance will be paid
