Key Result Areas: Responsibility for Staff Participate and maintain the effective communication of information and data between team members, patients and family networks and offer appropriate support and advice. Work at all times as part of the Orthopaedic team, liaising closely with senior colleagues and the outpatient team, and respond positively to feedback from preceptors / colleagues. Act as a role model to junior staff providing support where necessary. Supervising junior staff members as appropriate. Acting as mentor for students, learners, junior nurses and Health Care Assistants and recording progress, as appropriate. Participates in teaching in the clinical areas as required. Participates in the orientation of new staff. Supports/assists manager in the establishment of good working atmosphere, and in making cost effective/ efficient use of available resources. Acting as specialist/link person having undergone the relevant training sessions in your area of expertise. Leading by example to ensure that the rights, dignity and confidentiality of the patient are protected at all times. Undertaking any other duties that may reasonably be required. Organise own workload to ensure that the interests and the needs of the Virtual fracture clinic patients are met. Responsibility for Patients Assess, plan, implement and evaluate the care of clinic patients. Take responsibility to acquire necessary skills/ training to perform treatment relevant to area of work, such as wound care, applying splints and plaster casts. Provide a high standard of evidence-based nursing care for patients following the Trusts guidelines, policies and procedures. Take an active role in establishing and maintaining effective communication with all professionals, patients and relatives. Be responsible for the clinical observation and recording of data relating to patients in the department, ensuring that accurate records of patient care and progress are maintained and reviewed. Demonstrate sound judgement in assessment of patient care in order to establish any change in patients condition and contribute to the subsequent planning of care. To communicate via the telephone with patients in relation to their consultant prescribed care following the Virtual fracture clinic. Giving advice and ensuring that follow up appointments and advice leaflets are given as prescribed by the consultant. Maintain and protect the safety, confidentiality and dignity of patients and their families. Follow Trust policy for the care and custody of FP10 prescriptions when under their control. Be responsible for ensuring safe moving and handling techniques, making use of the appropriate aids available. Demonstrates a receptive and sensitive attitude to the needs of patients and relatives and deals with, or refers, any problems or complaints to the Department manager or Operational Manager. Responsibility for Clinics: Promote and ensure evidence informed practice. To practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain own NMC registration. To undertake the nursing assessment of care needs and the development and implementation and evaluation of programmes of care. Carrying out all relevant forms of nursing care to a high professional standard. Administering medication, complying with the nursing responsibilities under the Medicines Act (1968), the Misuse of Drugs Act (1971), and any other relevant Trust legislation. Maintaining an effective reporting system by observing and reporting verbally and ensure a high standard of record / case note recording which will include IT systems. Liaising and maintaining good relationships with other staff of all disciplines to ensure the smooth running of services for patients. Collect the relevant data on the patients discharged from ED the previous day. Provide accessible advice and practical support to patients and relatives as required. Work in partnership with the consultants to ensure the patients are effectively managed. The post holder will be responsible for documenting the patients journey from ED referral to the outcome of VFC. The post holder is to ensure the information given over the phone on the injury is clear and followed up with a letter to the patient and GP. The post holder should be able to challenge decisions made by more senior colleagues if the outcome does not comply with the VFC protocol. Assist with the application of walker boots as required and give professional advice on their use. Change casts as required. (Training will be given). Take clinical responsibility for patients within your allocated area of work. Communicate with relatives and other visitors. Prepare clinic rooms for procedures including preparation of sterile instrumentation, equipment and supplies as required, ensuring appropriate use of resources and minimal waste. Assist in the maintenance of good communications within the Trust. To act as a role model to other nursing staff. Assess, plan, implement and evaluate treatment, using appropriate programs/methods of care. To ensure delivery of high-quality care in accordance with Trust policies, procedures and guidelines. Participate in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives. Ensure medical records are available for planned procedures and liaise with medical records to provide an efficient service. As far as possible, maintains the necessary equipment and supplies in the department, in order to carry out nursing care with efficiency and safety. Responsibility for Managerial/Leadership skills: Supervise junior staff and address any developmental needs. Evaluate and report on nursing staff performance during their probationary period and at other times as requested. Participates in setting standards of care to ensure a high quality of service is delivered. Ensuring approved Trust / hospital clinical nursing policies / procedures are undertaken. To participate in Clinical Evaluation and Audit as appropriate. To liaise appropriately and promote collaborative working with other agencies such as Social Services and other community service providers Responsibility for Education To teach, as required, nursing students and junior members of the care team through demonstration and explanation and act as mentor as appropriate. Participate fully in clinical supervision and staff performance reviews. Assist with induction and in-service training of nursing staff. To maintain personal clinical expertise as required by the needs of the service and the NMC Professional Code of Conduct. Take responsibility for own professional development and clinical update. Attend appropriate courses/study days as approved by the manager. Comply with policies/procedures and in particular, issues relating to Health and Safety, COSSH, Incident Reporting and the Data Protection Act. Responsibility for Resources: To take charge of the clinic and ensure its smooth running. Share responsibility for appropriate and cost-effective use of resources. Understands and participates in the procedure for ordering supplies within the agreed department budget as required. Responsibility for Administration Accountable for the provision of accurate and legible record keeping and documentation in line with NMC guidelines. Demonstrates an awareness of clinical governance, participating in risk management and quality assurance initiatives. Responsibility for Strategic and Service Development: Participate in the implementation of new ideas and changes in the department that will enhance the quality of patient experience. Participate in the clinical governance and risk management strategy for the department. Participate in Clinical Audit as required. Initiate and develop new ideas and changes within the Orthopaedic department that encourages staff participation and results in improve patient care (either directly or indirectly). Responsibility for Research and Development: Be responsible for maintaining an up to date registration on the NMC register. Practices in accordance with the NMC Code of Professional conduct and other appropriate NMC guidelines Effort, Skills and Working Conditions The post holder is required to be able to use a computer to access results and patient information. The post holder requires the skills to use patient handling equipment and to assist doctors during outpatient procedures and the nursing skills required for fracture clinic. The post holder may be required to stand for long periods of time. The post holder should have a high level of concentration, be able to work around interruptions and prioritise workload. The post holder should be able to support the patient and relatives during consultation and when patients are receiving bad news. The post holder may, at times, have to deal with aggressive, upset or frustrated patients and remain calm at all times.
