To manage a caseload of adults with learning disability referred into the service with unmet health needs using evidence based and person centred principles to assess, plan, implement and evaluate creative and dynamic nursing interventions. To screen and prioritise new referrals on behalf of the service. To plan and facilitate training to groups of adults with learning disability and carers on a range of health promotion topics including long-term medical conditions, healthy eating and their sexual health. Contribute to and support planning, development and roll out of specialist community learning disability nursing services and pathways for people with learning disability across Lambeth, Southwark and Lewisham. To assess the health and wellbeing needs of individuals with learning disability allocated to caseload. To build a comprehensive case history through conversation with the person, their carers, colleagues in primary care and use of available clinical systems. To plan, deliver and evaluate programmes and nursing interventions to address the identified health and wellbeing needs of those individuals. To screen new referrals in accordance with current standard operating procedure. To facilitate and lead groups of adults with learning disability to learn about their health needs and develop confidence and skill to self-manage long-term conditions. To raise awareness of good health and wellbeing through health promotion activities (Individuals and groups) To monitor the health and wellbeing of individuals and groups and contribute to protecting those groups and individuals whose health and wellbeing is at risk. To carry out Nursing Procedures appropriate to role. To recognise the significance of observations made and act autonomously making decisions in isolation when working with people in their own homes and the community, in potentially challenging and unpredictable situations. To use clinical judgement around the circumstances in which adult and child safeguarding referrals are needed, and make those referrals in a timely way. To co-ordinate the provision of clinical care, within a defined caseload To advocate on behalf of people with learning disability To participate in and contribute to various clinical meetings To maintain up to date client records in accordance with service policy and NMC requirements. To be legally and professionally accountable for own actions and act in accordance with NMC Code of Professional Conduct. To ensure that due regard is given to the customs, values and spiritual beliefs of patients and colleagues and that principles of equal opportunities are practiced.
