An exciting opportunity has arisen for a full time, permanent Band 7 Inpatient Diabetes Specialist Nurse. The successful candidate will be based in the Diabetes Education Centre at Chesterfield Royal Hospital and work within an established multidisciplinary team of Diabetologists/Endocrinologists, diabetes specialist nurses and dietitians to provide specialist diabetes care to Inpatients throughout the trust including critical and emergency care areas. The role will include working alongside the existing Diabetes Inpatient Specialist Nurse and Inpatient Diabetes Support Worker.

To provide an autonomous inpatient nursing service for patients with diabetes whilst in hospital using advanced clinical skills and knowledge of the speciality

To manage the nursing caseload for patients with diabetes, who have multiple and complex needs including complex Type 1 and 2 diabetes, diabetes technologies including continuous glucose monitoring and hybrid closed loop insulin pump therapy.

To provide education to patients on self management of diabetes at home including home blood glucose monitoring, insulin administration and the potential risks and complications of living with diabetes.

To take responsibility for own evidence-based practice and implement and add to the body of knowledge that supports nursing practice within diabetes.

To lead and innovate diabetes related nursing practice, both within the Trust and through local networks.

To support all staff within the trust on issues related to diabetes management and therapies.

To actively contribute to and participate in the delivery of Trust-wide multi-disciplinary mandatory training and teaching.
