Solutions4Health are delighted to be offering the role of Service Coordinator - (Smokefree Bristol) at Bristol. This is a full time permanent job with a salary of £28,000 to £30,000 depending on experience. Role Summary Smokefree Bristol is Bristol Borough Council’s commissioned community stop smoking service to support residents of the Borough. As the Stop Smoking Service Coordinator, you'll be at the forefront of the service engagement with partners, stakeholders and local communities. As a coordinator, you will also be responsible for your own client caseload working with service users on a121 basis to support them on their journey to becoming smokefree and lead healthier, smoke-free lives. This dynamic role involves coordinating and delivering innovative smoking cessation programs, working closely with clients to provide personalised support and guidance. You’ll be responsible for the day-to-day coordination of the Smokefree Bristol service and will lead on service coordination across the borough. You'll also collaborate with healthcare professionals and community partners, driving engagement and ensuring the highest standards of care. This is an exciting opportunity for a dynamic individual to demonstrate their broad health and wellbeing knowledge, skills and experience in the delivery of exciting stop smoking service in Bristol with revised and client centred model of support. MAIN DUTIES AND RESPONSIBILITIES Ensure that all targets, performance measures, and outcomes are achieved successfully for Smokefree Bristol Service Organise and coordinate project delivery and awareness events. Responsible for data collection for monthly and quarterly reports Research, locate, set up and review new clinic opportunities Work closely, and develop sustainable relationships with local partners, including Pharmacies, GPs, secondary care providers and workplaces in the development, promotion and delivery of the community Stop Smoking service. Liaise with the relevant authorities to obtain permission and consent for the delivery of marketing campaigns and events. Gather information on all referrals made to the service and follow up contacts accordingly. Undertake random spot checks in various clinics to ensure that service quality standards are maintained e.g. relevant literature, display materials, NRT products and completed Monitoring Forms are consistent and following best practice Manage your own caseload of clients successfully by achieving individual targets and KPIs with a view to lead the team by example Co-ordinate the recruitment and training of staff by helping the service manager Ensure that local residents interested in smokefree Bristol, can access the service using means and tools suitable to them Ensure that printed materials providing information and advice on how to quit smoking are made available to residents in their first language where English is not tgheir first choice Co-ordinate and deliver, the provision of VBA (Very Brief Advice) training to partners and frontline staff as per service KPIs Encourage service users and other stakeholders to be involved in the design, ongoing improvement and evaluation of our services Investigate, report, and resolve all client queries and complaints by seeking guidance and support from service manager where needed Proactively maintain own professional knowledge, development and practice, and contribute effectively to team management meetings Work within professional boundaries, always maintaining safety and appropriate confidentiality Explain the principles of Stop Smoking, including self-efficacy and behaviour change, so that service users can make informed choices regarding their treatment and engagement pathway Deliver a client-led treatment plan in accordance with national and local guidelines, effectively and professionally, including Carbon Monoxide monitoring, accurate data entry to comply with NICE guidelines, and a comprehensive account of weekly interventions. Assist with carrying out the external quality control testing all devices and ensuring that all equipment is appropriately calibrated within timescales Ensure that the Quality standards are maintained within the service Maintain promotional resources and materials up to date Knowledge of promotional media channels to regularly post motivational messages and client feedback, success stories About Us Solutions4Health are an innovative company with a passion and a track record for tackling health inequalities.
