This is a key strategic and operational role at the heart of South Sefton PCN. You will:
• Provideclinical leadership, supervision, and mentorshipto nursing and healthcare staff across the PCN.
• Offerregular clinical supervision sessionsto nursing colleagues, supporting reflective practice, professional development, and ensuring high standards of clinical care.
• Deliver evidence-based nursing services to patients, working autonomously and in collaboration with the clinical team.
• Support the clinical team in developing treatment plans for chronic disease patients and identify, manage, and support patients at risk of developing long-term conditions.
• Foster a collaborative and supportive environment forpractice nurses, working to unify and strengthen the nursing community across practices.
• Manage the Housebound Chronic Disease Management programme, ensuring high standards of care and service delivery.
• Work closely with the PCN leadership team and practice partners to design, develop, and implement future clinical programmes and transformation projects.
• Deliver patient care that is safe, effective, and person-centred, adhering to clinical best practices and national guidelines.
• Support quality improvement initiatives, service development, and innovation in long-term condition management.
• Act as a clinical resource, role model, and mentor to nursing colleagues across the network.
• Prescribe medication in accordance with PGD and within your scope of practice.
• Provideclinical supervisionto nursing teams within the PCN, ensuring safe, effective, and reflective practice.
• Promote consistency in nursing standards and practice across PCN member practices.
• Coordinate and supervise on chronic disease management for housebound patients, including care planning, reviews, and proactive case finding.
• Encourage shared learning and continuous professional development through regular engagement with nursing teams.
• Develop and maintain effective communication pathways between clinical and administrative teams.
• Contribute to the planning and delivery of new PCN initiatives, aligned with national and local priorities.
• Use data and clinical insight to evaluate outcomes and drive service improvements.

We are now looking for an experienced and passionateSenior Nurseto join our multidisciplinary team and play a pivotal leadership role across our member practices.
We are looking for a motivated and experienced registered nurse with:
• Current NMC registration.
• Non-Medical prescriber registration
• Significant post-registration experience, ideally in primary care or community settings.
• Strong leadership andclinical supervisionskills, including mentoring and development of clinical staff.
• A proactive, collaborative approach with excellent communication and interpersonal skills.
• Confidence in managing long-term conditions and understanding of the challenges facing housebound patients.
• A passion for improving patient outcomes and nursing practice across organisational boundaries.
