Crynodeb
The following are the core responsibilities of the Practice Nurse. There may be occasions when other duties are required, depending on workload and staffing levels: a. To support the Infection Prevention and Control (IPC) lead by assisting with audits, training, and providing evidence of compliance to the practice manager as required. b. To develop, implement, and embed an effective care management programme for frail and housebound patients registered at the practice. c. To assess patients healthcare needs, ensuring the delivery of appropriate, evidence-based nursing care. d. To design, implement, and promote health and wellbeing programmes tailored to the needs of the patient population. e. To implement and evaluate individual treatment plans for patients with chronic diseases, promoting health and wellbeing. f. To identify, manage, and support patients at risk of developing long-term conditions, aiming to prevent adverse health outcomes. g. To provide routine nursing care to patients as required, following clinical evidence, NICE guidelines, and the NSF. h. To diagnose and manage acute and chronic conditions as necessary. i. To provide wound care, including ulcers and Doppler assessments. j. To review patient medications to improve compliance. k. To advise patients on the use of prescribed and over-the-counter medications, including side effects and dosages. l. To request pathology tests and manage pathology results appropriately. m. To oversee chronic disease clinics, delivering patient care and referring to secondary or specialist services as required. n. To maintain accurate clinical records in compliance with relevant legislation. o. To ensure effective use of SNOMED CT codes by all members of the nursing team. p. To ensure the maintenance and appropriate use of chronic disease registers. q. To develop, implement, embed, and participate in well-woman clinics. r. To develop, implement, embed, and participate in well-man clinics. s. To ensure compliance with the practice chaperone policy. t. To assist GPs with minor surgery as required. u. To prioritise health issues and intervene appropriately. v. To support the clinical team in managing emergencies. w. To recognise, assess, and refer patients with mental health needs. x. To implement vaccination programmes for adults and children. y. To support patients in the use of their prescribed or over-the-counter medicines within own scope of practice. z. To liaise with external services and agencies to ensure appropriate support for vulnerable patients. aa. To delegate clinical responsibilities appropriately, ensuring safe practice and that tasks are within the scope of practice of the individual. bb. To support the clinical team with safeguarding matters in accordance with local and national policies. cc. To understand and apply practice and local policies relating to substance abuse and addictive behaviours, referring patients appropriately. dd. To deliver opportunistic health promotion when appropriate.
