Summary

The following are the core responsibilities. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Work within the NMC Code of Conduct; maintaining professional standards of practice and behaviour for nurses. Prioritise people, practise effectively, preserve safety and promote professionalism and trust. Recognise and work within own competencies. Diagnose and manage patients with long term health conditions integrating both drug-and non-drug-based treatment methods. Implement and evaluate individual specialised treatment plans with patients with long term health conditions. Work with patients in order to support compliance with and adherence to prescribed treatments. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing as indicated. Review medication for therapeutic effectiveness, appropriate to patient needs, and in accordance with evidence-based practice and national and practice protocols (within scope of practice). Discuss with GP if any medication change is indicated (if not a non-medical prescriber). Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health. Provide health promotions and encourage well-being; using skills such as motivational interviewing. Document accurate recalls following reviewing patients with long term health conditions. Liaise with administration team regarding recalls for patients with long term health conditions. Provide other clinics as required by the surgery (within your scope of practice and competence) For example Smears, ECG, Acute and Chronic Wound Management, Doppler Studies and Phlebotomy. Administration of S/C or IM medication as prescribed; for example prostap & hydroxocobalamin. Run annual influenza clinics and provide opportunistic influenza vaccines during other clinics as indicated and adhering to PGDs. During clinics, check if patients are up to date with the national routine immunisation schedule. Offer vaccinations to eligible patients in accordance with the national routine immunisation schedule and adhering to PGDs; for example pneumococcal vaccination. Chaperone patients as indicated. Undertake the collection of pathological specimens, for example swabs & urine samples. Request pathology services as indicated. Recognise patients presenting with mental health needs, assess and take action required. Discuss with colleague or GP if concerns identified (within your competencies). Contributing to the monitoring and maintaining of the cold chain including contributing to the twice daily record of fridge temperatures, report any breach of the cold chain and take action as per cold chain policy. Work within the national standards of infection control and cleanliness and regulatory professional requirements and good practice guidelines. Prioritise own workload and ensure effective time-management strategies are implemented. Acting as a supervisor to students and newly qualified staff as required (within your scope of practice) Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual). Contribute to practice targets; for example QOF, complying with local and regional guidance. Maintain the knowledge and skills you need for safe and effective practice. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development. Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences). Take part in clinical supervision. Continually review clinical practices, responding to national policies and initiatives where appropriate. Report all incidents including near misses. Participate in the review of significant and near-miss events. Take part in the development of practice clinical protocols Take part in audit cycles. Attend nurse meetings on a regular basis. Attend other meetings as requested, for example community meetings. Monitor and ensure the safe storage, rotation and disposal of medicines Develop working relationships with external organisations and develop awareness of services available to surgery population. Signposting as indicated. Participate in local initiatives to enhance service delivery and patient care Demonstrate due regard for safeguarding and promoting the welfare of children and adults.
