- Key Responsibilities
- Lead and manage the sourcing of care placements, ensuring timely hospital discharges (within 3 hours of optimisation) and community referrals.
- Manage and support junior staff, ensuring high standards of service delivery.
- Provide expert advice and guidance to colleagues, stakeholders, and service users.
- Investigate and respond to complex enquiries and escalated complaints.
- Drive improvements in placement processes and procedures.
