Crynodeb
Direct clinical assessment and care provision of people in their home environment, who have been identified as meeting the Virtual Ward admission criteria. Co-ordination of daily Multi-Disciplinary Team meetings and Consultant ward rounds, ensuring that patient related information is up to date and information is disseminated accordingly. Act as a point of contact providing advice, support and guidance to colleagues and other professionals on issues relating to patient care. Communicating sensitive information concerning patients medical condition(s) and status which involves relationship building; displaying persuasive and reassurance skills acting in the best interests of patients at all times. Demonstrating skills to assess and interpret specialist acute and other patient conditions within the patients home environment; initiating actions as appropriate Due to the key requirements for this role, applicants must have the means to travel between multiple addresses.
