Crynodeb
1.To understand and follow good practice within the framework for the Mental Health Act, Mental Capacity Act and DoLs and Care Programme Approach. 2.To maintain accurate records, both written and computerised to a standard that reflects their level of clinical expertise. This may involve the use of specialised psychometric tools to complement the assessment and evaluation of service user care. 3.Liaise with community based services and resources such as Focus, housing, Police and voluntary sector organisations to facilitate the delivery of the most appropriate and best possible care for the individual service user, carers and their families. 4.Discuss issues surrounding risk management and plan care accordingly advising others regarding management of care with senior practitioners. 5.Discharge planning. 6.To respond to service user crisis in a positive pro-active manner. 7.To follow clear care pathways, including methods to reduce the risk of further crisis developing. 8.To work flexibly within a 24 hour rota covering early, late and night shifts according to service user need. 9.To provide telephone support to any service users, carers, outside agencies or clinicians who require information, advice or crisis intervention. This will require the Post Holder to make immediate, effective decisions that will have implications for the positive outcomes of the service users mental health. 10.Undertake comprehensive CPA mental health assessments then develop and implement care packages in a variety of settings. 11.To assist in maintaining safe custody of drugs in accordance with the Trust policies and procedures. 12.To cover 7 days a week, 24 hours a day on a rota.
