Act as the lead case manager for patients in D2A (mainly P2) independentsector beds placements including the operational and clinicalcommissioning for D2A beds in the independent sector, coordinatingongoing review, discharge planning, and MDT involvement. Reviewing request for Independent sector D2A beds and SPOT purchasebeds, including mental health via STOC referral from the discharge hub. Reviewing funding requests for 1:1 packages of care following a dischargeinto a D2A Independent setting including the on-going review of the 1:1package to the point of discharge. Conduct in person 1:1 reviews of patients receiving 1:1 care withincommissioned P2 (Discharge to Assess) beds; and ensuring all decisionsfollowing a review of 1:1 packages of care are made jointly via an MDTformat with the relevant health and social care professionals including theD2A independent setting providers. The rationale for any decision madeshould be transparent and clearly documented using the relevant proformasand ensuring all stakeholders are informed of the outcome. Assess the ongoing appropriateness and clinical need for 1:1 support, usingclinical frameworks, behavioural assessments (e.g. incidents, any triggers,evidence review of behaviour) and multidisciplinary team input.
