Responsibilities Act as a team member in the delivery of practice nursing services, ensuring that the needs of the patient are a priority Managing the nursing care of patients suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. Accurate and efficient input of data for contract purposes is also required Assist the whole practice team to help deliver effective implementation of management of patients and their needs. Contribute to infection control measures within the practice according to local and national guidelines. Responsibility for engaging with annual appraisal of personal performance, and engaging with feedback as appropriate Take responsibility for own learning and performance including participating in clinical supervision and acting as a positive role model Actively promote the workplace as a learning environment, encouraging everyone to learn from each other and from external good practice Contribute to the development of policies - protocols - guidelines within the practice. Aid the practice team with implementation of new initiatives in the delivery patient care and services Clinical practice Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care Undertake opportunistic screening when clinically required. Experience in chronic disease management or willingness to undertake training Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of nursing practice. Work with patients in order to support compliance with and adherence to prescribed treatments Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Operate within the setting of the GP facility and in patients homes Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately. Undertake annual training and demonstrate competency in anaphylaxis and resuscitation techniques Cervical Screening As part of theNational Cervical Screening Programme, our Practice Nurses: Performcervical smear teststo detect early signs of cervical cancer. Providepatient education and reassurance, addressing any concerns about the procedure. Ensure accurate record-keeping and referral for any abnormal results. Child Immunisations Our Practice Nurses are responsible for administering routine childhood vaccinations to protect against serious illnesses, Other Responsibilities: Wound Care & Dressings Managing minor wound care and simple dressings. Contraceptive & Sexual Health Offering guidance on contraception, STI testing, and reproductive health. This includes monitoring, stock control and documentation of controlled drug usage according to legal requirements where appropriate Undertake mandatory and statutory training requirements Contribute to the application of infection-control measures within the practice according to local and national guidelines Where appropriate, contribute to policies relating to the storage of medications, hazardous substances and infection control, and updating in line with current guidance, monitoring and audit Where appropriate, contribute to policies that reduce environmental health risks, are culturally sensitive and increase access to health care for all Managing information Use technology and appropriate software as an aid in planning, implementation and monitoring of care, presenting and communicating information Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes Manage information searches and audit using the internet and local library databases Understand responsibility of self and others to the practice and primary care trust regarding the Freedom of Information Act Collate, analyse and present clinical data and information to the team Learning and development Where appropriate, undertake mentorship for more junior staff, assessing competence against set standards Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (eg courses and conferences) Assess own learning needs and undertake learning as appropriate Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning Abide by the legal requirements and statutory rules relating to practice and record, and maintain periodic professional registration with RCN & NMC Complete all mandatory training modules as specified by the organisation Equality and diversity Promote diversity and equality of opportunity, and contribute to a non-discriminatory culture Support people who need assistance in exercising their rights Ensure adherence to local chaperoning policies Act as a role model in the observance of equality and diversity good practice Accept the rights of individuals to choose their care providers, participate in care and refuse care Assist patients from marginalised groups to access quality care Undertake any training required in order to meet the needs of the practice
