To support and implement care that meets the mental, physical, spiritual,psychological and social needs of the patient, being sensitive to age, culture,race, gender, ethnicity, sexuality and disability. Take a lead in facilitating the day to day functioning of the Clozapine Clinic, withsupport and guidance from qualified professionals such as the nursing team,consultants and pharmacists. To monitor the effects and side effects of Clozapine medication in the community,with guidance from relevant professionals, such as the nursing team, consultantsand pharmacists. To administer intramuscular injections and monitor the effects of these, with theknowledge of contraindications and side effects, being able to relay accurateinformation to patients in a format that they are able to understand in accordancewith SOP0031.AFC 06/08/19 JME-335-19 Community Mental Health Nurse Band 5 To undertake venepuncture in the context of Clozapine monitoring, after relevanttraining and ongoing guidance. To undertake physical health checks, interpret results and respond to and reportconcerns appropriately. (No moving and handling required). Assessment, Care planning and Risk Assessment in line with Trust Policy. To be able to identify and respond to concerns appropriately. The ability to make a judgement about a patients mental health or wellbeingneeds and act appropriately to manage needs / risks. (For example, supportingself-care needs by helping a patient to organise their prescribed medication into adosette box, or reporting a concern / risk to a qualified member of staff). To maintain the safe storage of medical equipment within the clinical setting, inline with Trust Policy. To monitor clinical stock supplies levels and support in the maintenance ofadequate stock levels. To take responsibility for maintaining the safe use of clinical equipment usedwithin the role. To contribute to the provision of a safe clinical environment and promote thedignity of patients. To carry out team Audits with support as required, such as the Hand WashingAudit. To prioritise workload based upon the patient needs / service demands. To support multi-disciplinary meetings and contribute to information sharing andclinical decisions regarding service users in the care of the CMHTs. To support patients whose health and wellbeing is at risk, acting upon andreporting concerns appropriately. To have the ability to accurately communicate information to patients in a formatthat they are able to understand. To demonstrate understanding of the distress and emotional impact caused bymental illness for patients and their carers, as well as the impact upon their socialand personal circumstances. To adopt a non-judgemental approach with patients based upon respect anddignity. To assist patients, as required, to obtain support from agencies such as welfarerights, employment support, housing and substance misuse services. To assist patients to develop skills for everyday living i.e. budgeting money,shopping, cooking, establishing or improving social networks, accessing localservices etc. To have the ability to identify limitations and to be able to seek assistance andguidance from senior staff as necessary. To ensure the safety, wellbeing and interests of patients, staff and visitors.AFC 06/08/19 JME-335-19 Community Mental Health Nurse Band 5 To have an awareness of and follow Local and National policies and proceduresin relation to the clinical role. To demonstrate an understanding of the psychosocial impact that mental illnesscan have on service users and their carers to be able to promote their wellbeing. To support care programmes, reviewing and modifying care programmes inconsultation with the multi-disciplinary team. To support multi-disciplinary meetings and contribute to clinical decisionsregarding patients in the care of the Community Mental Health Teams. To work within Pennine Care NHS Foundation Trust policy, professional andlegal frameworks at all times. This requires risk assessment skills, awareness ofboth Pennine Care and Local Authority child and adult safeguarding protocols,relevant mental health legislation and the Care Programme Approach. To keep a high standard of written communication, accurately recording eventsand observations in accordance with Pennine Care NHS Foundation Trustpolicies, and the NMC code of professional conduct. To develop and understanding of and demonstrate the skill in the supervisionprocess, using your own supervision and the provision of supervision to juniorstaff and students. To develop mentorship skills, and to provide professionalsupervision to and the assessment of student nurses. To account for your own professional practice in accordance with the NMCprofessional code of conduct, and to maintain the requirements for regular reregistration. To keep up to date with current trends in mental health care government policyand research based practice with particular relevance to this service user group. To recognise and respond appropriately to challenging behaviour in line withTrust policies.To develop and demonstrate skills in the supervision process, both for personalbenefit and in supporting junior staff and students.
