Care Coordinator – Domiciliary Care
Location: Eastbourne
Salary: Competitive

Are you a compassionate, organised, and proactive individual with experience in domiciliary care? We are looking for a dedicated Care Coordinator to join our growing team and play a key role in delivering high-quality home care services.
Key Responsibilities:
Coordinate and schedule care visits in line with client needs and staff availability
Conduct risk assessments, care needs assessments, and prepare personalised care plans
Liaise with clients, families, carers, and external professionals
Ensure compliance with CQC standards and internal policies
Maintain accurate and up-to-date care records and staff rotas
Support recruitment, induction, and supervision of care staff
Provide on-call support on a rota basis
Requirements:
Previous experience in domiciliary care or a similar care coordination role
Strong organisational and communication skills
Ability to work under pressure and meet deadlines
Good IT skills and familiarity with care management systems
Driving licence and access to own vehicle preferred
NVQ Level 3 in Health & Social Care (or working towards) desirable
