Crynodeb

Clinical: Provide specialist medical care, advice and support in order to provide high quality individualised care to patients/clients in line with local and national guidelines. Provide medical assessment and care for all patients on the inpatient unit, in line with the patients care plan, as part of the multidisciplinary team ensuring that medical care is evidence-based (as far as is possible) and best practice. Work together with other members of the medical team to ensure each patient is reviewed as frequently as their symptom management dictates (usually at least once per day). Participation in weekly inpatient MDT meeting if working hours include this. Holistically assess patients admitted to the inpatient unit, planning initial management in discussion with the available senior doctor as appropriate. Carry out basic clinical procedures as necessary including phlebotomy and cannulation as necessary. The post holder will not be expected to attempt procedures in which they do not feel competent. Maintain accurate and timely patient records. Medical records are electronic based (Systmone). Ensure legible prescribing in accordance with local policy. Liaise with primary and secondary care services (and other statutory and voluntary agencies) as appropriate to ensure continuity of medical care. Complete holistic assessment records (including drug reconciliation charts, VTE screening), discharge letters, letters notifying death, TTO prescriptions and anticipatory prescribing in line with local guidance. (As appropriate within the hours worked.) Confirmation and certification of death in line with hospice protocol, liaising with the Medical examiner service and Coroners Office where required by law and local practice. Liaise with family members/carers as appropriate.Out-of-hours, maintaining principles as above, additionally: Undertake emergency admissions, in agreement with the out-of-hours consultant Re-assess, review and adjust treatment of any inpatient where necessary due to change in patient condition Teaching and Training: Participate in the induction for new members of staff. To take the opportunity to take part in in-house teaching and training if appropriate.Clinical Governance: To always adhere to the GMC code of professional conduct. To work within St Margarets Hospice organisational policies and to implement policies within area of responsibility. To work with other members of the team to ensure that evidence from research is systematically considered and practices changed as appropriate. To actively contribute to audits and review of service if hours allow. To complete relevant sections of mandatory training or show evidence of completion elsewhere. To commit to continuing professional development To participate in annual appraisal programme and job planning process as appropriate for job role. To keep appropriate records to comply with General Medical Council requirements for revalidation. To comply with the incidence reporting process and support service improvement.Other responsibilities To work within Hospice policies and procedures to ensure safe and effective working practices for self and others (e.g. Health and Safety, COSHH, emergency procedures, the Mental Health Act, Child Protection procedures) To always maintain patient confidentiality within Hospice policy and procedures.
