Chronic Disease Management: Oversee long-term condition management, including medication reviews and optimisation for patients with conditions such as hypertension and lipid disorders. Education and Support: Participate in patient education, provide training to colleagues, and support innovation in clinical research. Operational Efficiency: Manage prescription processes, respond to medication-related inquiries, and support quality improvement initiatives such as QIPP and QOF.
