To be responsible along with other nursing staff for assessment, planning, implementation and evaluation of all care aspects of individual patients care. Attending to the physical and psychological needs of the patients under the care of the nursing team. Act as key worker and co worker for a designated group of clients. Delivery effective individualised care by supervising staff in the assessment of patients condition. Ensure that accurate and objective information is recorded and signed by the appropriate member of staff. Establish therapeutic relationships with individual clients, relatives and carers. Write and contribute towards the formulation of care plans. Ensure effective and confidential communication, at all times maintaining strict confidentiality with regard to any information concerning patients and colleagues. Demonstrate an awareness of the needs of carers of patients with mental health problems. To fully comply with the requirements of the NMC and upkeep of registration. Be aware of the Mental Health Act 1983 and be aware of clients detained under section of the Mental Health Act 1983 in the clinical area. Supervise the formulation of plans for the patients discharge and transfer from the clinical areas in line with Care Programme Approach, Community Care Act 1990 and discharge policy. Observe any changes in a patients physical condition or mental state and document in an appropriate manner. Assist patients towards self-care and independence. Understand the need for personal responsibility in relation to using the correct techniques for moving and handling patients. Act in a relaxed manner, thereby contributing to the creation and maintenance of a calm and relaxed atmosphere/environment. Establish positive professional and appropriate relationships with patients adopting a non-judgemental approach. Always be prepared to listen and treat each patient with respect and courtesy; valuing them as individuals. Carry out planning and organisation of the therapeutic and social activities for patients in the clinical area. To act as a mentor to student nurses. To provide managerial supervision to junior colleagues. Participate in therapeutic and social activities/groups. As appropriate and necessary, assist patients who have the need to use hoists and wheelchairs. As requested, take on clinical, managerial and environmental responsibilities. Ensure the local security protocols and procedures are adhered to at all times. The post holder will be able to demonstrate insight into their own developmental needs and will formally discuss these in supervision and yearly appraisals. Where development is necessary the post holder must show commitment towards development. The post holder, having attained appropriate courses, must ensure that feedback and the opportunity of sharing of knowledge is given to their colleagues. There may be opportunities for the post holder to present informal and formal teaching sessions of both a clinical and academic nature. The post holder will be a practice assessor to learner students and will demonstrate the appropriate skills to objectively assess. It is a requirement that all band 5 staff complete training as a practice assessor. The post holder will be expected to contribute to audit and research at ward level. Positive role model to colleagues. Liaise effectively with other multi-disciplinary team members and the residents/patients significant others. The post holder will be expected to be involved in standard setting at ward level and to be active in the evaluation of the same by the use of audit tools. Complete mandatory training in line with the Trust Policy Receive an annual appraisal and provide annual appraisals to lower banded staff Provide training relevant to HCA and lower banded staff Take all allocated annual leave Receive management supervision and provide management supervision where appropriate Comply with the CPD, practice hours and revalidation process requirements set out by Nursing Midwifery Council (NMC - This is the essential requirement for yearly registration) Medicines competency framework Mentorship and supervision of students and junior staff Receive clinical supervision and provide clinical supervision to Bands 3 / 4 Supervise and manage on a day-to-day basis lower banded staff within the professional group and team locality as required and in line with Trust HR processes. To participate in the staff appraisal process and to undertake for any staff you supervise Ensure lower banded staff are appropriately inducted, receive appropriate supervision, have an annual appraisal and are compliant with mandatory training requirements Attend seminars / conferences and other CPD events external/internal Attend regular key Trust-wide meetings / forums i.e. Professional nurse advisory committee, nurse forum, Link nurse forum (Infection control) Attend additional in-house developmental training, and support delivery of a number of mandatory training e.g. STORM, TASI, Infection Control, Safeguarding Complete Preceptorship within 6 months of commencement of employment in professional nursing role Resilience in dealing with emotive service user issues. To identify own stressors and take appropriate action to prevent negative impact of this on wellbeing and seek appropriate and timely support
