This is a collaborative role hosted by Filey & Scarborough Healthier Communities Network (FSHCN) supporting patients across four Practices: Scarborough Medical Group, SSP Health Hackness Road, Filey Surgery and Hunmanby Surgery. The post supports the increasing demand on primary and secondary cancer care services by ensuring a well-coordinated, person-centred approach that integrates health, social, and voluntary care sectors. Cancer patients, particularly those who are housebound, frail, or living with long-term conditions, represent a vulnerable population needing enhanced, tailored support. Filey and Scarborough healthier communities network is committed to developing an integrated workforce and delivering innovative models of care. This role builds on our proven success in supporting vulnerable patients and enhancing the patient journey. Job Summary The Cancer Care Co-Ordinator will serve as a central point of contact for cancer patients, supporting them from diagnosis through to treatment, survivorship, or end-of-life care. The role involves developing and maintaining cancer registers, coordinating cancer care reviews, signposting to appropriate services, and ensuring that personalised care and support plans are developed and followed. The role requires close liaison with Macmillan Wellbeing teams, community nursing and therapy services, hospices such as Saint Catherine's, and local authority services.
