Crynodeb
Liaise with patients, community nursing and social care teams, GP practice staff, GPs, GP practice pharmacists, community pharmacists, care home staff, falls prevention team and secondary care discharge co-ordinators and pharmacy teams in all aspects of technical medication compliance reviews. Attend staff meetings/handovers to identify patients suitable for medicines optimisation technician intervention. Support the implementation of changes required and agreed by the GP with regard to dosage, formulation of product, supply of compliance aids. Record actions and recommendations in patients notes/clinical management plans and inform their GP. Provide clinical advice and training to patients/clients and carers/staff in accordance with evidence-based guidance e.g. NSFs, NICE Guidance or Local guidelines. Ensure effective communication to patients about changes in how their medicines management needs can be best met and document it in the care plan. Prepare reports from the technical review and feedback to the relevant members of the healthcare team.
