CLINICAL: Under the supervision of the Team Manager support the qualified and unqualified staff in the provision of a high quality clinical service. Work with other services aiming to reduce health inequalities and improve access to universal services. Support access to mainstream health services for adults with learning disabilities. Provide timely intervention aimed at promoting and maintaining the health of adults with a learning disability. This will be achieved through individual and group therapy and where necessary the implementation of therapeutic interventions. To work with colleagues of all disciplines in the provision of a multidisciplinary health and social care service. Support the MDT to undertake assessments for people with learning disabilities who may have complex physical, psychological, behavioural or mental health needs and to develop and deliver person centred, individualised treatment plans. To work to national Physiotherapy standards, guidelines and strategies including those affecting the Allied Health professions services as agreed within the service. In conjunction with the CLDT Manager and Registered Physiotherapists, assess, problem solve, review and discharge patients following agreed assessment criteria, protocols and clinical care pathways. In conjunction with the CLDT Manager and Registered Physiotherapists liaise with and offering advice / information to other members of the multi-disciplinary team, other professionals, clients and their families/carers to facilitate safe and effective care. To follow research and evidence based practice as they are introduced to the service including National Institute for Clinical Excellence (NICE) guidelines to achieve therapeutic goals. To be clinically responsible for all aspects of the post holders own practice and develop and maintain the skills required to practice at this level of physiotherapy. To maintain a high standard of clinical practice including the ability to reflect and feedback to the CLDT Manager, physiotherapist or other team members highlighting any noticeable or significant changes and monitor treatments as requested. To actively support adults with learning disabilities in their use of local health services. Identify any inadequacies and take appropriate action to enhance the quality of intervention and access available. Establish and sustain links with primary care settings and non-statutory agencies facilitating access to universal services for adults with learning disabilities. To motivate others through effective communication and support the Physiotherapist to gain co-operation for treatment where there may be significant barriers to understanding. NOTE: Due to the increase to the minimum salary sponsorship threshold to £25,000 per annum, entry-level Band 3 roles will no longer be eligible for international sponsorship.
