Crynodeb
Contribute to consistent and reliable Occupational Therapy provision across all Acute ward settings. To support and assist wards in planning and supporting the admission of service-users with comprehensive physical health needs including assessment, intervention and care-planning. To manage own time constructively and prioritise clinical workload working with clients on a 1:1 basis. To work in partnership with team leads, managers and other practitioners in the effective management of referrals, case prioritisation and coordination within clinical care pathways. To plan for robust, sustainable discharge and aftercare, involving clients and their families in this planning and evaluating the care given. To contribute to service-development and quality improvement projects across Acute and Urgent Care Service Line. To contribute to specific clinical audits. To be part of the discharge planning discussions where appropriate across sites. To support and deliver training in relation to client needs. To participate in, and lead where appropriate, in-service training within OT and the wider service. To contribute to the development of the Occupational Therapy framework at South West London and St Georges. To provide clinical supervision to Band 5 Occupational therapists and university placement students.
