Description
We are seeking a dedicated and compassionate Social Worker - Adults 1 (Grade 8) to join our team. This role involves working with adults who require support in various aspects of their lives due to physical, emotional, or mental challenges. As a Social Worker, you will engage with individuals and families to assess their needs, develop care plans, and connect them with essential resources and services. Responsibilities
Conduct comprehensive assessments to determine the needs and circumstances of adult clients.
Develop and implement personalized care plans in collaboration with clients and their families.
Provide ongoing support, counseling, and guidance to clients to help them navigate their challenges.
Advocate for clients' rights and access to resources, services, and opportunities in the community.
Collaborate with healthcare professionals, community organizations, and other agencies to coordinate support services.
Monitor and evaluate client progress, making adjustments to care plans as necessary.
Maintain accurate and timely documentation of client interactions and case management activities.
Requirements
Bachelor's degree in Social Work or related field; Master's degree preferred.
Current social work license or eligibility for licensure in the relevant jurisdiction.
Strong understanding of adult services and community resources.
Excellent communication and interpersonal skills to build rapport with clients.
Ability to work independently and as part of a multidisciplinary team.
Proficient in case management and documentation practices.
Commitment to continuous professional development and staying informed about social work best practices.

End Time: 17:00
Pay Per Hour: £28.14
