Crynodeb
These roles are to support citizens to achieve their goals, promote independence, health and well-being throughout the ‘make safe and stabilise’ and/or reablement support period.
Provide person-centered, high quality support to citizens in their own home or in other appropriate locations, to maintain and develop daily living skills that will enable full participation in a range of activities which support customers to access appropriate community services.
Provide practical, personal and intimate care whilst maintaining high levels of hygiene and health and safety.
Work with residents in goal setting, as detailed in the support plan to enable the resident to maximise their independence and rehabilitation through reablement programmes and emotional support. To contribute to future support planning by monitoring, evaluating and communicating with colleagues supporting the citizens journey, external partners and stakeholders.
Establish citizen needs and think innovatively to deliver solutions utilising local community groups/ resources to support the cisizen to achieve their goals.
Complete all documentation and correspondence accurately and to a high standard in line with procedures and within agreed timescales, noting progress made and any changes of significance to the health, safety and wellbeing of residents to relevant colleagues and stakeholders.
Work in line with National Care Standards, Care Quality Commission and City Council policies and procedures, incorporating changing legislation and best practice. Maintain a thorough understanding of safeguarding procedures with the ability to recognise and respond appropriately to any issues that arise and participate in any investigations as appropriate.
Proactively participate in new initiatives and future changes in service delivery.
The role holder will provide practical and emotional care and support to citizens ensuring adherence to agreed standards to ensure positive outcomes for service users, maximising independence and opportunities.
The role holder will work with service users, their families and other relevant stakeholders to offer advice and support to help citizens develop and achieve their goals.
