Provide pharmaceutical services to designated Care Homes, supported by a Clinical Pharmacist, to ensure residents are managed safely and medically optimised. Conduct medicines reconciliation from medication administration record (MAR) chart reviews on transfer of care within Care Homes. Assist in the implementation of personalised care and support plans for patients in Care Homes. To support our Clinical Pharmacists to provide medication reviews on targeted groups of patients, including reconciliation and synchronisation of medicines. To undertake relevant clinical audits to ensure patient safety is maintained and medicines optimised, and act upon the results liaising with patients and/or practices. To monitor national medical and patient safety alerts via Eclipse to alert and support member practices, as needed, to ensure actions are completed in a timely manner. To support practices in maintaining CQC standards for medication safety. Conduct appropriate clinical/physical assessment during face to face clinics e.g. BP, pulse and pulse oximetry (within approved competence). Conduct medicines use reviews with patients in a supported living environment. To support national cardiovascular targets through early detection and optimisation of treatment for lipids in face to face clinics (within approved competence). To undertake thorough reasoned analysis on possible medications switches which maintain the quality of therapy of the original drug with a lower average unit cost. To manage and prioritise their own work across a range of practices. Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines. Inform patients regarding changes to their medicines in a professional manner and have the capacity to support and reassure patients when necessary, dealing with any issues or concerns that may arise within the boundaries of the post. To record all relevant pharmaceutical information in the patient's medical notes. Carry out housekeeping whilst in the patient record e.g. medicines synchronisation. To review prescribing processes which maintain or increase patient safety but also eliminate waste and duplication. To write, review and update standard operating procedures (SOPs) as the service evolves. To support the training and supervision of foundation trainee pharmacists.
