Crynodeb
Have responsibility for the assessment, planning, delivery, and evaluation of evidence-based quality care to an allocated group of patients on a shift basis. Work in partnership with patients and those that matter to them, involving them in decisions about their care. Recognise any deterioration or change in a patients condition which may require the intervention of others and refer as appropriate. Work collaboratively as part of the multidisciplinary team, establishing and maintaining effective communication and relationships. Provide early intervention to ensure the safe discharge of patients, working in collaboration with other relevant teams and services to prevent unnecessary readmission of patients. Act as an advocate for the patient and their family, protecting their interests and statutory rights, whilst promoting privacy, equality, diversity, and inclusion.
