Crynodeb
You will work within Shropshire’s People’s Directorate, with children, adults and families. The role will involve working with children on the edge of care, children in care and supporting children and young people to remain in, or return to, the care of parents and family where it is safe and, in their interests, to do so through support to the adult.
As an Outreach Worker you will assist the delivery of intensive family support to children and young people and their families where:
There is a risk of family breakdown (Edge of Care). An Assessment of Needs identifies support to the child/young person and/or family which may prevent an unnecessary reception into Local Authority care.
There is a need to offer a young person additional support to make a successful move into independent living from a care arrangement.
To help a young person stepdown from foster care or residential placement back to the family home.
To support a parent with changes they need to make to enable a child/young person return home
