Liaison Psychiatry Senior Clinical Practitioner William Harvey Hospital

Join our well established and multi-disciplinary Liaison Psychiatry team who are diligent in creating effective care plans and comprehensive clinical strategies focused at delivering exceptional hospital recovery and care for people with acute mental health need.

We welcome applications from highly motivated and experienced senior psychiatry practitioner who are capable of building and supporting teams of practitioners with effective decision making and communication skills. Ability to facilitate timely interventions and provide specialist assessment that will enhance best outcome and recovery for adults (18years+) is vital in this role.

Ideal candidate will be an experienced /registered mental health nurse, Occupational therapist or social worker with post registration experience of working in mental health. You must have completed practice supervision, be educated to degree level and have or be working towards a level 7 Masters qualification.

Accept referrals of patients aged eighteen years and over for assessment, treatment and coordination with other relevant services.

Work in collaboration with all members of the Liaison Psychiatry Service and involved agencies.

Maintain a clinical caseload of patients with co morbid physical and mental illness.

Provide specialist clinical input, liaison, consultancy and education to colleagues.

Develop and provide evidence-based treatment plans for identified patients, in collaboration with the patient and referrer.

Formulate, implement and evaluate treatment programmes.

Promote the wellbeing and mental health of people, by providing relevant information and advice to them, their families and to supporting agencies.

The post holder will provide advice and support on clinical management and assessment of risk.

The post holder will provide advice on the management of patients subject to the MHA and MCA.

The post holder must hold a full, valid, UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

To assess patients who attend the hospital with a known or suspected mental health problem, including those with dementia or delirium. Share expertise of mental health crises in over 65s. Work in close partnership with the Acute Hospital Trust, and Kent and Medway Partnership Trust teams in order to promote good working relationships and effective clinical care. Ability to assess where social need is greater than mental or physical health need and work with colleagues to coordinate appropriate plans of care for patients. Demonstrate risk management, and safeguarding issues, practicing safely and in accordance with individual patient need. To be able to formulate a comprehensive, multidisciplinary, integrated plan of care and communicate to relevant parties in a timely fashion. Involve carers at every step when appropriate.

Assess and anticipate individual needs of patient in crisis, taking appropriate action and communicating to relevant agencies/parties as required. Report all incidents as per the Trust incident reporting guidelines and policy.

The post holder will have the ability to work autonomously when carrying out assessments and planning care and collecting data.

To work within the Kent and Medway NHS and Social Care Partnership Trust Hospital Liaison Psychiatry Service to deliver a hospital focused service for people with co morbid illness

To develop links with and work collaboratively with Acute colleagues, and with those in the Hospital Psychiatry Liaison Service to enhance the interface between the specialist service and general hospital service.

To provide a specialist assessment and intervention service for adults (18yrs +) with co morbid illness and hold a small complex clinical caseload.

To act as an educational and consultative resource for colleagues working within the general hospital, primary care and the hospital liaison psychiatry service.
