South West London Breast Screening Service is looking for a highly motivated and competent Screening Tracker. You will work closely with our Data Manager and will be working as part of a dynamic multidisciplinary team and will provide comprehensive administrative support within a busy environment. 1.Proactively track all patients called to screening assessment and escalate as required to ensure a timely diagnosis and treatment for cancer patients, in line with national cancer targets. 2.Liaise closely with those involved in providing timely care for patients, working closely and proactively with the clinical teams to ensure that the screening pathway progresses smoothly and in a timely manner. 3.To hold and to also maintain knowledge of the wide range of procedures involved in booking appointments, to effectively record patients onto Cerner and Infoflex. 4.Accurately input and update assessment screening data to the local Cancer Wait Teams relevant database for the Screening Unit to report on patient tracking and adherence of the Faster Diagnosis Standards pathway and 62 Day Cancer Wait Times 5.Attend the weekly PTL Meeting and the Screening MDMs to ensure tracking information is accurate and up to date; and following up on actions as appropriate in conjunction with the Breast management team. 6.Maintain timely and accurate data collection, taking corrective action when data is incomplete or inaccurate. 7.Proactively identify and report delays at all stages in the patient pathway, highlighting any relevant issues to the Data Manager so that we can work together with our clinical colleagues to ensure sufficient capacity to meet the agreed patient pathway standards. 8.To maintain training requirements relevant to delivering the role in line with local and national targets and applications used; to support the service by training new starters on use of relevant applications used to deliver the role. 9.To proactively monitor the SWLBSS Referral log, liaising with Data/MDM Coordinators and referral administrators; ensuring associated referral reports obtained and sent as part of the referral process to external treating trusts. ***Please refer to attached job description and person specification for full job main duties:***
