Crynodeb
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks this will be dependent upon factors such as workload and staffing levels Deliver diabetes clinics, including routine reviews and follow-up appointments. Undertake comprehensive diabetes assessments, including, blood pressure monitoring, blood glucose and HbA1c reviews. Initiate and adjust diabetes treatments within a patient-specific directive (PSD) or under GP guidance, including insulin titration. Provide structured education for newly diagnosed and long-term patients Support lifestyle management including diet, physical activity, and smoking cessation. Promote patient self-care and shared decision-making in diabetes management. Liaise with multidisciplinary teams (GPs, dietitians, podiatrists etc) for coordinated care. Participate in QOF (Quality and Outcomes Framework) monitoring and data collection. Maintain accurate, timely documentation in accordance with NMC and practice standards. Stay current with NICE guidelines and emerging diabetes care pathways. To provide general practice nurse duties in line with professional competence Monitor and ensure the safe storage, rotation and disposal of medicines Support and participate in shared learning within the practice Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach
