We are looking for Registered Nurses to join a community nursing team in Worthing.


The Community Staff Nurse is responsible for working without direct supervision in a number of community settings to follow the nursing process and deliver appropriate nursing care to patients on the caseload. The postholder will also be an information resource for patients and colleagues and work collaboratively with our health and social care partners.

The Community Staff Nurse works in partnership with the primary health care team, social services, private and voluntary sector, addressing health promotion priorities both locally and nationally.

Please note we will be offering two part time roles - 0.6 WTE each.

To deliver high quality care in the community, working with patients towards rehabilitation, self-management and independence throughout the continuum of care.

To participate in a team of colleagues supporting evidence based care, facilitating staff learning and development.

To work flexibly across teams and/or transfer between teams to meet service needs.

Our main duties of the role:

a) Holistic assessment and care planning.
b) Individualised care and the promotion of self-care.
c) Safe and effective review and referral to the appropriate member of the multidisciplinary team.
d) Effective delegation and prioritisation of workload.
e) Professional development as required to maintain area of practice, and as negotiated in supervision.
f) An awareness of health, safety and security risks
g) Engagement in service provision and change.
h) Acts as a practice supervisor for staff and students.
