Practice Nurse / Nursing Associate Location: York Medical Group Variety of Hours Available Full-time & Part-time Considered Reports to: Clinical Manager Are you a passionate Practice Nurse or Nursing Associate looking to make a difference in primary care? Whether you're experienced or newly qualified, if youre committed to promoting health, managing chronic conditions, and providing high-quality evidence-based care we want to hear from you. What You'll Do Deliver personalised care through assessments, treatment plans, and health education. Manage chronic disease clinics and screening programmes (e.g. cervical cytology, vaccinations). Conduct medication reviews and contribute to prescribing where appropriate. Support women's health, travel advice, and immunisation services. Lead on health promotion and patient education initiatives. Maintain high standards of infection control and clinical documentation. Contribute to audits, QOF, LES/DES targets, and practice improvement plans. What Were Looking For Registered Nurse or Nursing Associate with valid NMC registration. Confident in managing long-term conditions and preventative care. Skilled in patient assessment, clinical decision-making, and documentation. Experience or interest in health promotion and chronic disease management. Strong team player with excellent communication skills. Willingness to work flexibly across sites and contribute to a dynamic, multidisciplinary team.
