JOB SUMMARY To assess, develop and implement specialist palliative care for patients and family of those diagnosis with cancer and other life limiting illness, monitoring and evaluating the effectiveness of that care enabaling patients to be maintained in the environment of their choice. Provide and lead specialist education to others to promote best practice in Palliative Care. Communication with others Patient and families and significant others Members of Primary and neighbourhood Health Care Teams Health and Social Care colleagues at all levels within the community, hospitals, hospice and independent settings Palliative Care Team members Supportive Care at Home Service Co-ordinator Statutory and voluntary agencies KEY RESULT AREAS: The postholder will: 1.1Care/service/function provision: To manage own specific geographical caseload, and day to day management of colleagues in absence of team members. To give highly specialised advice, information, guidance and support to patients, families, Neighbourhood teams, Primary Health Care Team, Allied Health Care Professionals and Social Care Professionals. To provide highly specialist clinical leadership for patients and their families to meet individual palliative care needs in the community, working independently with significant discretion. To autonomously assess, plan, implement and evaluate the care for individual patients and their families to meet individual palliative care needs in the community, working independently with significant discretion. Where it is appropriate to the service, upon successful completion and registration of the relevant qualification with the individuals professional body and SWYPFT to function as an independent non-medical prescriber and carry out relevant CPD to maintain competence. To develop effective and empathetic supportive relationships with patients and their families, using advanced communication skills to relate highly complex, contentious, sensitive information taking into account barriers to understanding and challenging communication. To monitor and identify complex bereavement issues and refer to appropriate services. To act as a resource in developing and signposting information for patients and staff. 1.2 Working as part of a Team: To act as Palliative Care representative at primary care Multi-Disciplinary Team Meetings. To be a core member of Palliative Care Multi-Disciplinary Team meeting and neighbourhood team. To initiate, design and deliver palliative care education programmes for multi professional groups to promote high quality palliative care. To act as a mentor to newly appointed clinical nurse specialists in palliative care setting To contribute to business planning for service development and strategic planning in palliative care services To develop network and regional contacts enabling the sharing and promotion of new developments in Palliative Care. To participate in Team responsibilities for efficient management of Palliative Care Services, including the accepting, recording and safe keeping of donations from the general public for Macmillan and Home Care Support Fund. To participate in the formulation and implementation of policies and procedures and evaluate standards of care within palliative care services. To develop and lead integrated audit systems, which influence and guide palliative care practice in line with Department of Health Guidelines. To participate in national research projects. To maintain accurate manual and computerised records to ensure continuity of care and facilitate accurate data collection for service development 1.3 Managing Self: To critically evaluate and disseminate existing research to promote evidence-based practice incorporating into SWYPFT Clinical Governance Guidelines. To participate in professional development and clinical supervision programmes. To be aware of and act in accordance with Trust clinical standards, guidelines, Health and Safety and work within the NMC Code of Conduct. To have developed flexible self-management skills and have the ability to concentrate where the work pattern is unpredictable. 1.4 Management Supervision and Support To be part of the core neighbourhood team and provide leadership for palliative/ end of life care team for the team. Safeguarding: Report any concerns regarding the safety or wellbeing of children, adults service users, members of their families etc, in accordance with Trust Policy. Prevent and respond appropriately to abuse and understand own role in this by undertaking Safeguarding training. 1.8 Trust policies and commitments to be read in line with the relevant Trust Policy: All staff employed by the Trust must comply with the Trusts policies and procedures, undertake appropriate training required for their role and commit to: ensure they are aware of the Whistleblowing Policy and how they raise concerns; maintain confidentiality, in line with the Trusts Confidentiality Policy and Code of Conduct; understand their personal responsibilities with regards to data quality for any information which they create, use or process in accordance with the Data Protection Act 1998 and Trust Data Protection Policy; comply with the provisions of The Trusts Health and Safety Policy and Protocol. Ensuring their own safety and that of colleagues, service users and visitors. Know the action to be taken in the event of a fire and must undertake fire training annually; receive supervision in line with the Trusts Supervision Policy and an annual Appraisal in line with the Trusts Appraisal Policy, during which mandatory, role specific and personal development needs should be identified and agreed; understand their responsibilities under the Trusts Equal Opportunities in Employment Policy and ensure that they adhere to the provisions of the policy; recognise, respect and support the equality diversity of staff, colleagues, service users, carers and the public. Contribute to a working environment which promotes and responds positively to difference and diversity; ensure they carry out their duty to safeguard and promote the welfare of children and young people under the age of 18 years, as issued under Section 11 of The Children Act 2004, by being familiar with and adhering to Trust safeguarding policies and participatingin relevanttraining; comply with their professional responsibilities to develop their practice and deliver care through a Clinical Governance framework (i.e. CPD, Audit, Supervision); demonstrate, through practice and practical understanding, the importance of the continual development of individual, team and service wide quality improvement; abide by relevant codes of professional practice, with the organisation taking action when codes of conduct are breached; work flexibly to meet the needs of the service/organisation, whilst working within a culture of progressive employment practices and commitment to the Investors in People (IiP) Standards. adhere to the Trusts smoke free policies, which prohibits smoking anywhere on Trust grounds; being socially responsible by complying with measures that support the Trust in reducing or offsetting our environmental impact; Identify and report risks, hazards, incidents, accidents and near misses promptly; In addition to the Trusts own responsibilities under the Health and Social Care Act 2008, Code of Practice on the prevention and control of infections and related guidance, for your safety, ALL staff (and contractors) are responsible for ensuring their work adheres to this Code in the delivery of safe patient care.
