The Integrated Community Rehabilitation Services (ICRS) in South Manchester Local Care Organisation are currently recruiting a Band 3 Rehabilitation Assistants who is passionate about providing care to residents in their own home. There will be the opportunity to work with a variety of services that make up the ICRS as assistants will rotate. These services include, Community Therapy Team, Community Falls, Post Covid and Community Neuro Rehab Team.
ICRS provides therapy for patients with a wider range of conditions who would may be struggling to access care in an out-patient setting. The successful candidates will work with a team of Physiotherapists and Occupational Therapist as well as the wider Multi-Disciplinary Team to provide treatment and intervention as prescribed by a qualified clinician.
The successful applicants will possess experience of working within health or social care teams, show good communication skills and be dedicated to providing high quality care to patients.
Applicants MUST have a UK driving license and access to a vehicle to travel across South Manchester to be able to carry out home visits. Please note that evidence of this will be required. If you are unable to provide this evidence you will not progress to interview.
Work with a diverse group for patients with complex medical conditions
Take an active part in the delivery and evaluation of evidence based home exercise and therapeutic programmes under the indirect supervision of a therapist
Work as part of a team to develop and maintain clinical skills and competencies
Work without direct supervision undertaking tasks delegated by a qualified health care professional
Support patients to achieve their goals and promote independence and healthy lifestyles
