Summary
About the Role
1st Grade Care Ltd is seeking an experienced and HCPC-registered Occupational Therapist to join our expanding clinical team. This is a community-based / potential hybrid position offering the opportunity to work directly with individuals in their own homes to promote safety, independence, and well-being.
The successful applicant will undertake comprehensive functional assessments, deliver intervention plans tailored to individual needs, and work collaboratively with multidisciplinary teams to support rehabilitation and safe living.
This role is suited to a motivated, autonomous professional with a commitment to high standards of practice and a strong understanding of community care frameworks in Wales.

Key Responsibilities
• Delivering person-centred, holistic occupational therapy assessments in service users’ homes
• Formulating evidence-based care and intervention plans aimed at promoting independence
• Recommending and facilitating housing adaptations, including the preparation of DFG reports
• Advising on and arranging the provision of specialist equipment
• Supporting discharge planning, falls prevention, and reablement programmes
• Liaising with health professionals, care coordinators, housing officers, and family members
• Maintaining accurate documentation in line with CIW and GDPR standards
• Contributing to service development, safeguarding, and quality improvement initiatives

Essential Criteria
• BSc (Hons) in Occupational Therapy (or equivalent recognised qualification)
• Current registration with the Health and Care Professions Council (HCPC)
• Minimum of 12 months post-qualification experience (community or local authority preferred)
• In-depth knowledge of relevant legislation, including the Social Services and Well-being (Wales) Act 2014
• Strong assessment, communication, and record-keeping skills
• Valid UK driving licence and access to a vehicle for home visits

Desirable Criteria
• Experience completing assessments for Disabled Facilities Grants (DFG)
• Familiarity with WCCIS or other electronic care management systems
• Background in reablement, mental health, or equipment provision services
