As part of community transformation we are changing the way we manage referrals. We have developed a referral and assessment hub and are looking to recruit B6 Mental Health Practitioner to support management of referrals and assessments within the hub.

You will support the triage completed by the senior practitioner and will jointly support the decision making on the onward journey for that service user. You will be aligned with one of our CMHT's but may work across all three to complete assessments.

You will provide duty assessments for the referral and assessment hub at the designated base or at a venue agreed with the service user and carer. You will need to be able to travel citywide across the Salford Footprint. There may also be times when that duty assessment will require you to inreach into the ward if they are an inpatient at the time of referral.

This post is a full time position (37.5 hours per week) and the working hours will be 9am - 5pm 5 days per week . The post is based at Cromwell House The post holder will be expected to travel across the Borough to attend satellite clinics across Salford.

Key duties will include:
• Triage of referrals coming into the referral and assessment hub as part of a team. Providing an onward journey if not appropriate for Community Services.
• Completing assessments for those people requiring specialist mental health services under Community Mental Health Team.
• Complete assessment for those inpatients who may require a secondary service upon discharge.
• To link in with the MDT and attend a weekly meeting to discuss your assessments .
• To promote awareness of the role of the referral and assessment hub, within local service area/team, GP practices and the wider community.
• Supporting service user, carer and stakeholder involvement
• Promote a culture of learning, education and professional development
• Championing quality assurance by establishing quality standards relating to the service user and carer experience, implementing Divisional Action Plans and undertaking complaint investigations
• Adherer to the principles of safeguarding of children and vulnerable adults in line with Trust Policy and local reporting procedure and ensuring junior staff compliance with this.
