Summary
We are excited to be able to recruit Frailty Support Workers to join our existing teams in the delivery of the Royal London Hospital’s Frailty service.
The Frailty Support Worker will support with the delivery of care for older adults with frailty, both within the hospital and within Tower Hamlets community. This fixed term post will include weekend working therefore the post holder must be available to work within a flexible rota.
We are looking to recruit a Healthcare Assistant to join our existing team of Specialist Frailty Practitioners (nurses and therapists) in delivery of the Royal London Hospital’s virtual and inpatient frailty services. This 12 month fixed term post includes weekend working therefore the post holder must be available to work within a flexible rota.
The Virtual Frailty Ward supports older patients living in the borough of Tower Hamlets in their own homes. You will be working alongside the Band 7 Frailty Practitioner and the GP and have regular MDT discussions. You will be supported in your learning and development by an expert team.
As part of your role, you will be visiting patients in their own home to take blood tests and observations. You will also have the opportunity to facilitate face to face assessments and will also support the team with administrative tasks.
This post forms part of the expansion of the Frailty Team within Older Peoples Services at the Royal London Hospital. The team includes doctors, specialist nurses and therapists who care for frail and elderly patients in the Emergency Department, the Acute Admissions Unit and throughout the hospital.
