Lead audits, implement MHRA alerts and NICE guidance, and support compliance with local formularies and shared care protocols. Review and authorise repeat prescriptions, ensuring appropriate monitoring and safe prescribing practices. 3. Integrated Care & Service Development Support the transition of care by reconciling medicines post-discharge and minimising medication-related hospital readmissions. Participate in multidisciplinary team meetings, care planning, and case conferences to ensure coordinated patient care. Identify cohorts of high-risk patients using clinical systems and support proactive interventions. 4. Education, Leadership, and Quality Improvement Provide training and advice to practice staff on prescribing, therapeutics, and medicines management.
