Crynodeb
To receive referrals by telephone or electronically which are recorded onto CCC digital patient tracker and EPR. To perform the initial checks of the electronic referral for comprehensiveness and accuracy. To gather a current holistic overview of the adults health and care journey. This will inform the decision making around the complexity and priority level of the referral. To ensure that general office duties are carried out, including the systematic filing of all general correspondence and patient documentation, photocopying and use of e-mail. To undertake the allocation / prioritisation of daily support for the MDT team
