Summary
Job Summary Care Navigator As a Care Navigator at Cornerstone Practice, your main responsibilities will include: Welcoming patients in person and over the phone, offering a calm, friendly, and professional first point of contact. Navigating patients to the most appropriate healthcare professional or service, using clinical protocols and your knowledge of local services. Managing appointments via our computerised system, ensuring efficient scheduling with doctors, nurses, and other staff. Handling patient queries, prescription requests, test results, and other health-related enquiries with tact, accuracy, and confidentiality. Supporting clinical teams with administrative duties, including maintaining accurate patient records, preparing clinic documents, and processing incoming correspondence. Responding to urgent situations, including arranging home visits or emergency services when needed, following agreed procedures. Liaising with external agencies, such as hospitals and community services, to support continuity of care. Maintaining a welcoming environment, ensuring patient areas are clean and ready for use, and that the building is opened and closed safely. Please see the attached Job Description for full details
