Crynodeb
12 bookable appointments per sessions. Mix of telephone and face to face appointments. Average of 1 day triage commitment per work. Communicating effectively with staff, colleagues and patients. Opportunistically addressing QOF alerts during consultations where appropriate. Contributing to QOF to maximise points in all areas. Identification and reporting of issues of quality and risk. Use SystmOne clinical system. All documents are subject to a robust workflow system. Checking and actioning results Protected time for your own reviews to allow for continuity of care. Minimal home visits due to having our frailty nurse at our attached care home and a dedicated frailty GP session for planned home visits. We are a supportive Practice and encourage discussion of cases and information at daily and monthly clinical meetings. Protected time for daily coffee meeting to discuss cases. Extended access cover is optional and can be done remotely if you opt in
