Job Responsibilities include but may not be restricted to the following Clinical delivery Clinical medication reviews Polypharmacy reviews Complex patient medication queries Care home and supported living support Care planning Clinical administration Review of clinical correspondence for medication management Medicines reconciliation Medication queries and requests Authorising repeat prescriptions Medicines information and advice Interpreting pathology Signing prescriptions (if qualified) Practice processes and performance Best practice prescription management including use of Repeat Prescribing Protocols Prescribing Quality Scheme implementation and support QOF optimisation and prevalence finding Medicines Management optimisation to support positive CQC outcomes Cost effective prescribing to support positive revenue generation Clinical Audits
