Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role…

We are looking for a Support Worker to join the Staying Well team.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

Thames Reach’s hostels are transitional spaces where a 24 hour staff team support people to move in, assess their needs, and quickly identify the best place for them to move on to next. Skilled staff teams develop personalised support plans to ensure people are ready to move on and are accessing support with their health, substance misuse, criminal justice and life skills needs

You will:

.Conduct regular assessments of service users’ needs to tailor support effectively and monitor progress toward their goals.
.Collaborate with internal departments and external partners to ensure the highest quality of care for service users.
.Develop and implement SMART goals (Specific, Measurable, Achievable, Relevant, Time-bound) to support service users in achieving meaningful outcomes.
.Promote engagement and social inclusion while fostering autonomy and independence to improve overall physical and mental well-being.
.Deliver a range of tailored interventions, both short-term and long-term, based on the individual needs of each service user.
.Maintain accurate and up-to-date records, ensuring all documentation and case files meet Thames Reach’s standards and compliance requirements.

You will have:

.Experience supporting individuals with mental health challenges, with a compassionate and empathetic approach.
.A strong prioritisation and time management skills, with the ability to work independently and efficiently under pressure.
.Proficient knowledge in Microsoft Office and database systems, with experience in tracking and managing information accurately.
.A detail-oriented and proactive outlook, ensuring high standards in both support delivery and documentation.
.Excellent interpersonal and communication skills, fostering positive relationships with service users and multidisciplinary teams.
.A genuine passion for mental health, with a commitment to making a meaningful difference in the lives of individuals facing both short-term and long-term challenges.
