Thames Reach runs projects and services that help to end homelessness in London. We work directly with homeless people on the streets and in hostels. We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role…
We are looking for a Support Worker to join the City Outreach team.
We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.
The Thames Reach Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond rapidly and work flexibly to reach people. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those offering immigration support.
You will:
.Be the first response to rough sleeping in the Square Mile conducting regular outreach shifts with volunteers.
.Assess the support needs of people found rough sleeping in a person-centred way.
.Manage a caseload of clients, and work alongside them to develop robust plans to end their rough sleeping.
.Develop a broad knowledge of local, regional and national support strategies.
.Work alongside partner agencies to ensure that clients have the necessary support to access and sustain accommodation.
.Work within databases to accurately record completed work.
You will:
.Provide a flexible and responsive service to people with experience of rough sleeping and homelessness.
.Have communication skills with the ability to form and maintain effective working relationships with partners.
.Have acommitment to supporting people to achieve the best outcomes as they work towards independence.
.Have strong organisation skills, with the ability to manage conflicting priorities and deadlines.
.Work autonomously as well as part a team.
.Have good IT skills with the ability use Outlook, online recording systems, Excel, and Word, using a computer and a smart phone.
