We are looking for a highly motivated individual to become part of our new exciting team, this role offers an opportunity to be part of a new community and hospital based model, the team will work closely with maternity services, providing administration and signposting services to families during the antenatal
and postnatal period. In this key role the successful applicant will navigate pregnant women and their families into local services throughout the critical 1001 days of life, offering seamless support and information aiming to positively impact life chances and reduce health inequalities within the service area.
This unique role offers the opportunity to work alongside community and hospital-based midwifery teams and local agencies to support families and provide accurate data on these services. This is a fantastic opportunity for the right person to broaden their current field of experience, expand existing skill sets and capabilities and work closely with both statutory and voluntary sector organisations within the district.
